Pod hasłem widnieje nazwisko i daty życia artysty: „Witold Chomicz 1910–1984”, zapisane białą, dekoracyjną czcionką. W dolnej części, mniejszym białym tekstem, podano miejsce ekspozycji: „zespół pałacowo-parkowy w Kozłówce” oraz poniżej: „Teatralnia – sala wystaw”.
